2026 Next Play 360° Parental Consent, Waiver, and Media Release
Participation Consent
As the parent or legal guardian of ("Participant"), I hereby give my full consent for their participation in any and all activities affiliated with Next Play 360°, including but not limited to: Next Play 360°, School of Athletics, The Lab Training, Homecourt Advantage, Beyond the Arc League, Camps, 360° Access Series, PUMA events, commercials, and any affiliated programs or activities.
Liability Waiver & Indemnification
I understand that participation involves inherent risks, including transportation to and from activities. I voluntarily assume all such risks and hazards and agree to waive, release, indemnify, and hold harmless Next Play 360°, its partners, affiliates, event sponsors, coaches, volunteers, facility owners, and staff ("Released Parties") from any and all claims or liability arising from Participant's involvement, except as may be covered by applicable insurance policies (e.g., Commercial, AAU, YBOA, or auto insurance). This waiver also extends to SCOOT HENDERSON, his family members, affiliated companies, and associated partners.
Health & Safety Acknowledgment
I acknowledge and accept the risks of illness, injury, or health conditions, including but not limited to COVID-19, associated with participation in the activities. I expressly release the Released Parties from any health-related claims.
Media & Appearance Release
I grant Next Play 360° and its affiliates full permission to record, photograph, and use Participant's name, voice, likeness, performance, and biographical or statistical information (collectively, the "Appearance") for promotional, commercial, and educational purposes. This includes use in print, online, broadcast, and all forms of media, whether now existing or developed in the future, worldwide and in perpetuity. I understand and agree that Next Play 360° shall have full rights to edit, modify, distribute, and use such content without compensation. All rights to the media content are solely owned by Next Play 360°.
Membership Freeze Policy
I acknowledge that if I need to temporarily freeze or pause my membership for an upcoming billing cycle, I must submit a written request by email to info@nextplay360.com before the applicable payment is due.

Submission of a freeze request does not automatically guarantee approval or prevent a scheduled payment from being processed. All freeze requests are subject to review and confirmation by Next Play 360°. Members are responsible for ensuring they receive confirmation that their request has been received and processed.
Failure to submit a freeze request in accordance with this policy may result in scheduled membership payments being processed as normal.
No Refund Policy
I acknowledge and agree that all registrations, membership dues, payments, and participation fees are non-refundable under any circumstances.
If a participant misses a scheduled training session, they may be eligible to attend a designated make-up session, subject to availability and Next Play 360° policies. Make-up sessions are offered as a courtesy and do not entitle the participant to a refund, credit, or cash reimbursement for any missed session.
Governing Law
This agreement is governed by the laws of the State of Georgia, and I consent to the jurisdiction of Georgia courts in the event of any dispute.
